Patient Policies

Please be sure to update your phone number, email, address and insurance information at every appointment.

  1. If you are a new patient or scheduled for an exam, please check your email for a patient questionnaire to get your updated medical history or we ask that you arrive 10-15 minutes prior to your scheduled appointment to complete in office.

  2. Please allow at least 24-48 hours for prescription refills to ensure that you run out of medication.

  3. If you any chronic medical issues (diabetes, hypertension, etc.) or have Medicare or a Medicare adjacent insurance, you may require a referral from your Primary Care Physician prior to being seen.

  4. Please allow at least one week for us to contact you with imaging results to allow our providers to review the results. We will contact you as soon as the results are available. (Specific to patients who receive testing when our providers are out of office. If you prefer to review testing results in office with the provider, please let our staff know and we will schedule accordingly.)

We strive to respond to all patients in a timely manner, however we do ask that patients allow at least 24 hours for our staff to return messages.

A comprehensive exam is intended to be a well visit that includes routine retinal screening through either dilation or Optomap retinal images. We do require patients complete one or the other at the time of their appointment to ensure that you receive full comprehensive care.

Co-payments are due at the time of service

No Show/Cancellation Policy: Please provide 24-hour notification to cancel an appointment. We understand that situations arise where this is not always possible, so we ask that patients remain mindful when needing to cancel or reschedule an existing appointment.
